The Community Rapid Response Service is a multi-disciplinary service made up of the following professionals:
- Registered Nurses
- Community Practitioners
- Advanced Practitioners
- Trainee Advanced Practitioners
- Allied Health Professions including Physiotherapy, Occupational Therapy and Pharmacy.
- GP
The service is provided in patients own homes including residential and nursing homes within the HMR catchment area.
Assessment in your own home by an appropriate clinician within 2 hours of referral.
Short term specialist intervention and treatment.
Access to Intermediate care beds, short term home care provision and hospital diagnostics as required.
Signposting and onward referrals to other specialist services.
Referrals can be made into the team from any heath professional.
The service provides short term intervention by NHS and social funded support to adults in crisis in order to avoid hospital admission and help service users reduce risk and remain independent in their own home environment.
